Participant Agreement

By submitting this registration, I acknowledge that I have read and agree to the Living Water Community Church, Inc. Event Registration Terms and Conditions, including the Liability Waiver and Media Release.

I understand and agree that:

  Participation in church events involves inherent risks, and I voluntarily assume those risks for myself and/or my minor child(ren).

  I release and hold harmless Living Water Community Church, Inc., its pastors, staff, volunteers, ministry leaders, and representatives from liability to the fullest extent permitted by applicable law.

  I certify that I (and/or my child) am physically able to participate in the registered event.

  In the event of an emergency, I authorize Living Water Community Church, Inc. to obtain necessary emergency medical treatment if I cannot be reached, and I accept responsibility for any resulting medical expenses.

  I grant permission for photographs and/or video of myself and/or my child(ren) to be used by Living Water Community Church, Inc. for ministry, promotional, and communication purposes unless I notify the church in writing prior to the event.

  I understand that registration fees are subject to the event's refund policy.

  If registering a minor, I certify that I am the parent or legal guardian and have the authority to complete this registration on the child's behalf.
